Before You Come In

In an effort to ensure your visit to Viverae Care Center is enjoyable, we ask that all new patients complete our New Patient Packet prior to your appointment. If you are a returning patient, please help us keep accurate information by completing the Returning Patient Packet prior to your next annual exam. You can find all of our forms here.

Cancellations

If you must cancel your appointment, please do so at least 24 hours in advance by calling the office at 469-398-3398. A fee may be charged for late cancellations.

Accommodation Information

If you are coming from outside the Dallas-Fort Worth area for your medical exam, you may need to stay in a nearby hotel. Please click here to view suggested hotels in the area, as well as a transportation information.

Appointment Instructions

Please do not eat or drink anything except plain water for 8 hours within the time of your appointment (to help ensure accuracy of fasting lab tests).
If you are taking medications or vitamin supplements, please bring a list or the actual bottles to your exam. Dosage is very important.
You should take your medicines the morning of your appointment if you do not require food to take them. If you are diabetic, ask the nurse about how best to handle your medications or insulin before your appointment.
Try to limit your exercise the day before your appointment to light exercise. You do not want to be tired for your treadmill stress test.
Do not use lotions on your chest the morning of the exam. The EKG electrodes tend not to stick properly.
If you are having a treadmill stress test, you will need to bring clothes and shoes that you can exercise in:

  • Men: T-shirt, shorts or warm-up pants, socks and athletic shoes
  • Women: Loose-fitting shirt or t-shirt, shorts or warm-up pants, socks and athletic shoes

Lockers and showers are available if needed.

Payment Options

Payment is expected at the time of service and can be made by cash, check, debit/credit card (American Express, Visa or Mastercard) and most Flexible Spending Account Cards.

Prescription Refill Policy

To refill our prescription, please call your pharmacy and ask them to have a refill request form faxed to the Care Center at 469-916-1824. Please anticipate the need for refill several days in advance so you will not risk running out of your medications. For some medications that require regular laboratory or other monitoring, it may be necessary to have a lab test or office visit prior to additional refills. Some medications cannot be refilled by phone and may require a written prescription to refill.

Medical Records Release

If you need to have medical records copied, please make your request as far in advance as possible. If you are requesting imaging or X-ray film, it will be necessary to obtain those from the imaging center where they were performed. Most specialists now want both the radiologist’s written report as well as a CD of the actual films or scan.
